1. Discogenic pain

Generally 80% of the population suffer low back pain in their lives. It can start occurring at as early as 3rd decade of their life. Most commonly Aging, obesity, vibration from occupation, smoking, excessive axial load accelerated disc herniation. These factors causes stress and reduce water content in disc, which causes degeneration and disc herniation and nerve root compression.

MRI can diagnose the disc herniation most commonly L4 L5 disc herniate followed by L5 S1.
It can be unilateral or bilateral, radicular or non radicular.

2. Neuropathic Pain

Neuropathic Pain occurs due to damage or injury or degeneration of the nervous system. It is often burning pain or shooting pain, stabbing or tingling kind of neck pain. Most of the time if injury to nerve become chronic causes neuropathic pain. Sometimes it is difficult to control. Commonly seen in severe lumbar canal stenosis after injury to spinal cord & nerve roots, it is diagnosed by MRI scan

3. Neurogenic Claudication

Neurogenic claudication featured by Pain, paresthesias, burning sensation in lower limbs from hip to toes, which aggravates on walking some distance, typically relieved by rest completely and starts again after walking. Slowly claudication distance increases as degeneration progress. This is due to compression of nerves in lumbar region. MRI done may show some spinal canal stenosis and compression of nerve roots.

4. Mechanical Instability Pain.

This pain is due to loss of integrity of spinal column or associated with instability of spine. Often in degenerative conditions like isthmic spondylolisthesis and in trauma it can happen. There may be low back pain which increases on walking, lifting weights, flexion & extension movements of the body. There may be difficulty in walking on toes & heels while examination. It is diagnosed by conventional X-Ray lumbar spine.
